Molar uprighting is a targeted orthodontic or surgical procedure designed to correct a mesially tipped or impacted molar, typically caused by the premature loss of an adjacent tooth. When a tooth is lost, posterior molars naturally tilt forward into the empty space, creating severe hygiene traps, localized bone loss, and traumatic occlusion. Uprighting the tooth repositions its long axis perpendicular to the occlusal plane, which eliminates food-trapping pockets and promotes periodontal health. This structural correction restores a parallel path of insertion, which is highly beneficial for the successful placement of dental bridges, implants, or partial dentures. Clinicians achieve this using fixed orthodontic appliances, continuous or segmented archwires, compressed springs, or temporary anchorage devices (TADs) to push the molar back up without extruding it. Alternatively, surgical uprighting offers a fast, single-visit solution where the tooth is mechanically repositioned and splinted.
