Death was supposed to be the end of suffering.

In Incartum, it is only the beginning.

Incartum is the vast plane of the afterlife, where the heavens, hells and forgotten realms of every faith exist alongside one another. Among them lies Aaru, the ancient Egyptian paradise of fertile fields, sacred rivers and eternal peace-a final reward for souls judged worthy.

But paradise is beginning to fail.

A mysterious affliction is spreading through Aaru. The dead are weakening, sacred protections are collapsing, and the realm itself appears to be decaying. With its gods unable-or unwilling-to stop it, help must be sought beyond Aaru's borders.

The search leads to Mysteria and to Veska, the infamous apothecary known for treating illnesses that should not exist among the dead. Yet this sickness is unlike anything she has encountered before. As its true nature begins to emerge, it becomes clear that Aaru may not be the only realm in danger.

Some diseases attack the body.

Others consume the soul.

And in Incartum, even death may not be enough to escape them.
